Online Coast Guard License Application Forms

This is a program to help and simplify the application process as you apply for your captain's license.

We at 3Bs have been appointed as "trusted agents" by the Coast Guard to screen applications, sea service forms, physicals and drug tests prior to submission to the Coast Guard.

Each section has our comments prior to the actual forms to assist the prospective mariner.

Step 1: set up your computer and printer

Step 2: read the comments first before each section

• Sea service form

Step 3: Fill out each form on the computer

Step 4: Print out each form

Step 5: Review the forms and make sure that everything is correct and filled out correctly.

Step 6: Bring all forms to your 3Bs class

The teacher will scan it for completeness and submit it to the Coast Guard for evaluation and license issuance. If it is complete and correct you'll get your license issued.

If there are any errors they will call you for more information.

In addition, if you've included an email address they will notify you by email as your paperwork progresses thru the evaluation and issuance stages.
